Welcome to the Einstein Health Office!
My name is Joy Nevens and I am happy to be a member of the Einstein Tiger Team. It is my goal to support student learning by addressing acute illness or injury, promoting health and wellness, minimizing the impact of health issues, and serving as a health resource to students and families.
I consider us partners in your student’s health and encourage you to keep me informed of any health issues or concerns that may impact your child while on campus. This may include new or changing medical conditions, mental health issues, medications, immunization updates, or any other health related concern.

Sports Physicals for Student Athletes
WIAA guidelines require that all students have a physical examination prior to competing in interscholastic sports.
The School District Sport Physical Form is valid for 24 months from the date of the exam.
If families do not use the Shoreline School District form, the healthcare provider and parent must provide all of the information asked for on the district form in order for the physical to be accepted by the school district. The health care provider must also write on their form that the physical they are providing is valid for 24 months.
Who may complete a physical examination as defined by WIAA rules?
• MD, Medical Doctor
• DO, Doctor of Osteopathy
• ARNP, Advanced Registered Nurse Practitioner
• PAC, Physician’s Assistant
• NP, Naturopathic Physician

Other forms are also needed before your student participates. These forms are in the Sports Packet available in the middle and high school main offices. These forms must be completed before each sports season. The physical will be kept in the school's health office.
Please remember: No nursing services are available during after-school activities. Emergency medications kept in the health office will not be accessible. If your student requires emergency medication, it is highly suggested that you and your student athlete personally discuss a plan with the coach.
